Roles & Responsibilities: PDTX (Per Diem Therapy Xperts) is a healthcare staffing agency hiring on behalf of our client for a Physical Therapist (PT) in Concorrd, OH (skilled nursing facility). Perform PT evaluations and re-evaluations; develop and carry out individualized plans of care; provide therapeutic exercise, gait/mobility and functional training; supervise PTAs as applicable; complete timely, compliant documentation; collaborate with the IDT and educate patients and families. Requirements: Active Ohio PT license.
